INSPECTORS said a Watersheddings care home requires improvements after an unannounced inspection was carried out.
"Hazards" and "immediate risks" were uncovered at Moorhaven Nursing Home in Ripponden Road by the Care Quality Commission (CQC).
The government watchdog reported unsafe bed rails, overhanging mattresses and infection control risks in their latest publication report.
However, the regulator rated the home as good for the care provision at the 33-capacity site which supports older residents.
A Moorhaven Care Home Ltd spokesman said they were pleased the good care was acknowledged.
The home is in the process of creating an action plan to address the concerns which were highlighted in the inspection.
Inspectors said in their report: "Hazards within the home were not always recognised and safety management systems were not robust enough to ensure safety for people.
"This meant people were at risk of harm.
"Documentation lacked the details required by staff to ensure residents received their medication as prescribed.
"We saw people had been provided mattresses on top of bed bases which did not fit safely and over-hung the end of the bed base meaning people were at risk of falling.
"We saw other hazards such ill-fitted carpets, uneven paving slabs and infection control risks where flooring in bathrooms was ill fitting."
The care home met the regulations which were in force at the time of its last inspection in August 2014 and secured a "good" rating for care provision.
Inspectors said: "People told us there were enough staff on duty to meet people's needs.
"There were positive interactions between people and staff, and people told us they were happy and felt very well cared for."
A Moorhaven Care Home Ltd spokeswoman said: "We are pleased the good care delivered at Moorhaven was recognised by CQC in the 'caring' section of the report.
"However, we are very disappointed the home has been given the same rating as the majority of nursing homes in Oldham, as we consider Moorhaven to be a 'good' home.
"We are in the process of devising an action plan to enable us to address the areas of concern at the next inspection. We feel confident that a home which has always enjoyed fully compliant ratings in the past will quickly improve, without any impact at all on our residents.
"We are an experienced provider of care homes and remain confident in the manager and staff at Moorhaven and the care we deliver to our residents.
"We have every faith in the regulatory system and we are confident we will move to an overall rating of good at re-inspection."
